Before your first visit, please take the time to download, print, and fill out our Patient Health History form.This will help ensure a speedy check-in. If you are not able to print our form, please arrive 15 minutes early for your appointment, or ask us to mail a form to your home. Download Health History Form

If you have had a full-mouth set of dental x-rays taken within the last three years, please bring a copy with you. Your previous dentist will be able to provide these to you.

If you find the need to change your scheduled appointment, please allow 2 working-days notice. This helps us to maintain a streamlined and on-time schedule.

Payment Policy:

We are committed to providing you with the highest quality dental care and up-to-date information so that you may fully participate in maintaining the best possible oral health. Our financial policy is intended to facilitate excellent service to you while minimizing our administrative costs.

- If you carry dental insurance, please bring the necessary information with you to your first appointment.

- Dental services are to be paid at each appointment with cash, check credit card or dental financing we have available. As a courtesy, we will bill your insurance directly for services received. Estimated co-payments and deductibles not covered by your insurance carrier are requested at the timeof service.

- Cash or check payments are eligible for a 5% discount. Visa and Master Card are accepted but are not eligible for discount.

- With the information your insurance company provides us, we will do our best to provide you an estimate of your co-pay prior to your appointment.

Please read your insurance benefit booklet and understand all waiting periods, frequency limitations, age limits any exceptions, and exclusions. If you are "double covered" with 2 insurance companies, be aware of a "duplication clause" and verify whether or not your secondary insurance has standard coordination of benefits or not.

- Pitman Family Dentistry offers a credit program through a third party agency CareCredit. CareCredit is a low, and in some cases, zero-interest credit card, which provides a flexible payment plan and can also be used for a variety of other health care services.
