First Time Visits

Walk, bike, or drive and park in our lot.
Check in with our receptionist. She will give you a few forms to fill out if you haven’t had a chance to complete them online before arriving.
Have your x-rays updated if you are due. (If you have current x-rays at another office, please let us know. We like to have them transferred for you before your appointment.)
Next you’ll meet the dentist. He or she will do an exam, discuss your dental condition, and present a plan to ensure your teeth stay healthy.
Before your first visit, please fill out the following forms. This will help ensure a speedy check-in. If you are not able to complete the form online, please arrive 15 minutes early for your appointment, or ask us to mail a form to your home.
X-Rays
If you have had a full-mouth set of dental x-rays taken within the last three years, you may bring a copy with you. Your previous dentist will be able to provide these to you. If you need help getting these transferred, let us know.
Payment
If you carry dental insurance, please bring your dental insurance card and ID with you to your first appointment. If you don’t have insurance, it’s no problem. Cash or check payments can be made and are eligible for a 5% discount. Visa and Master Card are also accepted, but are not eligible for discount.
Changing Your Scheduled Appointment
If you find the need to change your scheduled appointment, please allow 2 working-days notice. This helps us to maintain a streamlined and on-time schedule.
Insurance
Dental services are to be paid at each appointment with cash, check, credit card, or dental financing we have available. As a courtesy, we will bill your insurance directly for services provided. Estimated co-payments and deductibles not covered by your insurance carrier are requested at the time of service.
Payment Policy

Credit Program
Pitman Family Dentistry offers a credit program through a third party agency, CareCredit. CareCredit is a no, to low, interest credit card which provides a flexible payment plan and can be used for a variety of other health care services.
